Indians starting catcher Yan Gomes, who separated his right shoulder Sunday against the Twins, will be sidelined for four to eight weeks, the team announced.

He was hurt in the fifth inning Sunday after grounding to third and then being knocked off-balance by Twins first baseman Kennys Vargas' tag and falling awkwardly.

Though Gomes is hitting just .165/.198/.313 in 71 games, the Indians (54-38) have the lowest ERA in the American League.

His struggles at the plate gained attention over the weekend when the Indians performed a "sacrifice" to Jobu, echoing the baseball movie "Major League."

The Indians activated Roberto Perez from his rehab stint at Triple-A Columbus to replace Gomes. Perez,  0-for-8 with one RBI with the Tribe, required surgery for a broken right thumb suffered April 30 on a play at the plate against the Phillies.

He's batting .313 (5-for-16) with three RBIs in his rehab.
